Special Needs Housing
According to the Centers for Disease Control and Prevention, Georgia has one of the highest rates of new HIV diagnoses and mortality in the country. Union Mission has been at the forefront of this crisis since 1990. For more than 30 years, our Special Needs Housing Programs have empowered individuals trapped at the intersection of extreme poverty, homelessness, and the excruciating health impacts of HIV/AIDS. Our team ensures these neighbors have access to safe, stable housing, quality medical care, and other supportive services.
The Phoenix House is a 10-bed housing program for those living with HIV/AIDS that also offers short-term rental and utility assistance. Residents work with a case manager to develop an Individualized Service Plan that will guide them into independent living. Services include HIV/AIDS healthcare assessment and treatment, peer education, resource coordination, support groups, and substance abuse treatment.
The Phoenix House
The Tenant Independence Program offers permanent housing for adults and families living with HIV/AIDS at the various sites outlined below:
-Chatham County Tenant Independence Program (35 Scatter Site Units)
-Glynn County Tenant Independence Program (5 Scatter Site Units)
-Liberty County Tenant Independence Program (5 Scatter Site Units)
-Wayne County Tenant Independence Program (12 Scatter Site Units)
Tenant Independence Program
Daniel Flagg Villas
Daniel-Flagg Villas is a permanent housing community dedicated to special needs housing. The 10-unit community includes one-, two- and three-bedroom apartments. These apartments are leased to clients at an affordable rate according to each occupant’s income. Daniel-Flagg Villas allows clients who were once chronically homeless to live in safe affordable housing with continued access to supportive services.
Onsite Clinic
Residents receive assistance with medication adherence and disease management from a Licensed Professional Nurse (LPN) as well as transportation to and from offsite doctors appointments.
HIV/AIDS Awareness Education
The Phoenix Project provides HIV/AIDS Awareness Education throughout the Coastal Empire and offers free and confidential HIV testing to the community.
Counseling
The Phoenix Project has an onsite mental health and substance abuse counselor dedicated to educating the resident on the role and effects of drug usage and/or mental health diagnosis on their lives. The goal is to empower residents to create positive change in their lives.
